The word percent literally means 'per hundred'. So any percent can be written as that number over 100. 25% means 25 out of 100, or 25/100.
Write it over 100 and simplify
Put the percent over 100, then reduce to lowest terms. 25/100 simplifies to 1/4 (dividing top and bottom by 25).
Percents with decimals
For a percent like 12.5%, write 12.5/100, then multiply top and bottom by 10 to clear the decimal: 125/1000, which simplifies to 1/8.
Percents over 100
A percent above 100 gives an improper fraction. 150% = 150/100 = 3/2, or 1 1/2 as a mixed number.
